Digital Health Transformation: How ItaliAssistenza Evolved Patient Support Services with Mia-Care's Microservices Architecture

How ItaliAssistenza, a Zambon Group company, partnered with Mia-Care to build a white-label, cloud-native microservices platform unifying Patient Support Programs nationwide.

+200 healthcare professionals
+60k home visits completed
-67% time-to-market
-39% km per visit
+0.5% therapeutic adherence

ItaliAssistenza, a Zambon Group company, has been the leading Home Care Company in Italy for over 30 years. The organization operates nationwide through its three divisions:

Human AssistCare

Since 2006, it has been the Italian leader in designing and delivering patient support services — Patient Support Programs (PSPs) and home clinical trials — activated in partnership with pharmaceutical and biotech companies, medical device manufacturers, and CROs. PSPs are integrated physical and digital services designed to assist patients in therapy and their caregivers, aimed at improving therapeutic adherence and quality of life. They support the care pathway without replacing the National Health Service (SSN) and can include:

  • Nursing and psychological support

  • Technical assistance

  • Educational pathways

  • Home telediagnostics services

  • Drug delivery

  • Collection and disposal of medical waste

  • Monitoring apps for the patient and their physician

Through two operational centers (one operating 24/7), a nationwide capillary network of over 100 healthcare professionals, and a technology platform, more than 20,000 home visits and over 100,000 phone contacts are organized per year.

PrivatAssistenza

The first national home care network for the elderly, sick, and disabled. Today, with over 180 active centers across more than 2,000 municipalities, it is the leading national home care network, working alongside families daily to ensure high-quality services and continuous support.

careApt

Founded in 2018 by ZCube, the Zambon Group’s research venture, it became the first telemedicine and teleneurology clinic in 2019. Through a Healthcare Operational Center and a multidisciplinary team, it offers teleassistance and telerehabilitation programs to guarantee simple, continuous, and personalized access to care for patients and caregivers. Careapt delivers services in partnership with insurance companies, health funds, and corporate welfare programs.

The Collaboration in Brief

Mia-Care collaborated with ItaliAssistenza to develop a new white-label, cloud-native, modular microservices portal for managing services across the Human Assistcare and Careapt divisions. The portal ensures compliance with General Data Protection Regulation (GDPR), HIPAA, and international cybersecurity policies, enabling the secure and flexible configuration of data and operational flows.

This infrastructure guarantees seamless interoperability with existing internal and third-party systems through a documented API portal. The orchestration of multiple dedicated digital touchpoints synergistically involves all stakeholders: Operational Center operators, field operators, logistics providers, as well as patients, physicians, and partner companies. Furthermore, the solution is designed to support the development of software equivalent to and compliant with medical device standards.

ItaliAssistenza's Business Model: Enhancing Integrated Care with Technology

To deliver informational, organizational, and technical support services to patients, caregivers, and hospital physicians, ItaliAssistenza provides a structured system of home, telephone, and digital services. It adopts an operational model based on an integrated, multidisciplinary approach leveraging the following strategic assets:

  • A healthcare operational center supported by a multidisciplinary team

  • Two non-clinical operational centers, one of which operates 24/7

  • A field network of healthcare professionals

  • The strategic and constant use of technology

ItaliAssistenza continuously seeks initiatives to improve and innovate its services to maximize end-user satisfaction and streamline its operational model by increasing productivity and optimizing costs, thereby maintaining a competitive market advantage.

In this context, the need emerged to develop a new state-of-the-art technological and digital platform conceived to ensure synergistic, highly capillary management of all involved stakeholders: care recipients, caregivers, physicians, and operators.

The platform integrates various functionalities designed for continuous care, such as remote diagnosis and telemonitoring, leveraging digital channels (web portals and apps) to further increase the overall effectiveness and quality of offered services.

The Challenge: ItaliAssistenza’s Roadmap Towards a Digital Ecosystem

Industry demands, the pressing need to ensure data processing security and continuity, and the broad range of offered services led ItaliAssistenza to establish fundamental milestones for digitalizing its systems.

The main problem lay in managing dozens of different Patient Support Programs (PSPs), each with its dedicated CRM. Every instance entailed different configurations, different workflows, and separate databases, negatively impacting both operations and business:

  • Operations: No scalability or component reuse, high setup costs, extended onboarding times, lack of resource governance, vendor lock-in for reporting and changes, and complex integration with third-party systems.

  • Business: Slow setup, high recurring costs, and low competitiveness.

ItaliAssistenza’s Digitalization Objectives

To overcome this fragmentation, ItaliAssistenza set key objectives towards digitalizing and centralizing its systems:

  1. IT Infrastructure Enhancement: Develop a cloud-based platform with a robust, secure, and scalable IT architecture capable of evolving over time.

  2. Holistic Stakeholder Management: Ensure integrated management of all actors involved in service delivery (care recipients, caregivers, physicians, and operators) by providing cutting-edge features like remote diagnosis and telemonitoring.

  3. Patient Journey Personalization: Create integrated, easily adaptable Patient Journeys with a meticulously designed user experience for end users.
